Liability Release and Waiver

In consideration of my child/ward being permitted to participate in The Town of Kenly Parks

and Recreation sports programs and activities, I, the undersigned parent or legal guardian, acknowledge and understand that participation in athletic and recreational activities involves inherent risks, including but not limited to physical injury, illness, or accident.

 

I hereby voluntarily assume all risks associated with my child’s participation, whether such risks are known or unknown, and agree to release, waive, discharge, and hold harmless the Town of Kenly, its Parks and Recreation Department, officers, employees, volunteers, coaches, and agents from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that may be sustained by my child while participating in these activities, including travel to and from activities.

 

I certify that my child is physically able to participate and has no medical condition that would prevent safe participation, or that such conditions have been disclosed in writing. I grant permission for emergency medical treatment if necessary and accept financial responsibility for any medical care provided.

 

I, as the Parent/Guardian of the minor child being registered to participate in a youth recreation program with Kenly Parks and Recreation, give my permission for the use of my child's name and photographs in The Parks and Recreation Department’s publications both print and electronic- i.e. newspaper articles, social media posts, brochures and promotional materials.

By submitting this registration, I certify that I am the parent or legal guardian of the registered participant. I acknowledge that I have read, understand, and agree to the Liability Release and Waiver for Youth Athletics. I voluntarily consent to my child’s participation in all program activities and assume all risks associated with participation.

I further agree that this electronic acknowledgment constitutes my legal signature and has the same force and effect as a handwritten signature.

• Fees are due by the deadline date for registration to be complete.
• Registration fees must be paid before child can start practice.
• All fees are non-refundable (If there are not enough youth that sign up for specific sport/age group, a 100% refund will be provided.)
• Checks that are returned for insufficient funds will be assessed with a $25.00 service fee.
